Hospice at Home and Respite Care Services

Our Hospice at Home service is provided  by skilled and experienced Registered Nurses and Hospice Support Workers. The service is available to adults over the age of 18 who have a palliative diagnosis and   registered with a GP in North Northumberland and are approaching end of life. We work very closely with our colleagues in General Practice and Hospital Trusts to provide individualised care depending on clinical needs. Care is free at the point of delivery. We also provide support to carers of patients approaching end of life by offering respite overnight or during the day

Eligibility:

You need to be over the age of 18 years old and living within our clinical catchment area and registered with a local GP. Referrals to this service are made via your GP, District Nurse, Macmillan, or Healthcare professional – Just ask them to refer you.

We invite you to watch our short film featuring The Grey family talking about the impact of our end-of-life care for their Dad/Husband that enabled him to die at home, which was his wish.

Our Impact

"Your Hospice care team was professional at all times. Treated both my husband and I with the utmost respect and understanding. Made a very sad time bearable as they did everything they could to make my husband comfortable"
